  • Qualifications- This section is where you list your qualifications for serving as an expert panelist for this case. Remember NOT to include specific information that will allow someone to determine your identity. For example, do not list information such as "Chief of Thoracic Surgery at Seton Medical Center."
  • Issue(s)- Although you will not see the actual complaint unless the complainant has waived confidentiality, you will receive a thorough description of it. The Issue(s) section contains a brief statement of the issue(s) cited in the description of the complaint you receive. If you find an issue during your review that isn't cited in the complaint, you should not include it in this section. All issues should be included in the Standard of Care, the Application of the Standard of Care, and the Conclusion sections of your report. However, please remember that you should state whether each issue you find violates the standard of care.
