  • Facts– This section contains a detailed explanation of the facts of the case. It is important that you cite the facts of the case as accurately as possible. It is appropriate to include the pertinent negatives as well as pertinent positives of the case, however, don't include an opinion in this section. You do NOT have to list every patient contact, or visits to another provider, unless it is part of your determination of Standard of Care. Remember to cite the specific tab and page numbers of the medical records you are using to support the facts.
  • Standard of Care- This section states the appropriate standard of care as well as the basis and reasoning supporting the standard.
