  • Facts– This section contains an explanation of the facts of the case. It is important that you cite the facts as accurately as possible, however, this section does not need to be as detailed as the Facts section of reports in which the standard of care has not been met. Remember to cite the specific tabs and page numbers of the medical records you are using to support the facts.
  • Standard of Care- This section states the appropriate standard of care as well as the basis and reasoning supporting the standard.
  • Summary of Why Standard of Care Was Met- This section consists of an explanation of why you feel no violations of the standard of care exist. Although this section can be brief, it is important that you accurately convey your opinion based on the facts of the case and the standard of care you cited.
