  • Address the specific allegation(s) on which the description of the complaint you receive is based.
  • Determine whether any other aspects of the case not included in the allegation are not in keeping with the standard of care.
  • If imaging studies are an integral part of the case, personally review the relevant studies (if available) and comment on them in your report.
  • Limit your opinion to whether you feel that the standard of care has been met. Discussing other problems in healthcare delivery is not useful.
  • Be consistent throughout your report and avoid contradictory statements.
  • Be as factual and objective as possible when writing the report. List the care, the standards, and your conclusion(s).
  • Reference the appropriate Tab and page numbers containing material that supports your statements.
