• Please reference the specific portions of the medical records on which you are relying to make a given statement. The records will be available to you online through Laserfiche. They are separated by tabs, and each page is stamped with a seven digit number in the upper right corner(Bates #). You should refer to the designated tab and page number such as Tab 4, p. 0000025. If no Bates number is present, use the page number from the document preview screen.

If the report does not contain appropriate citations, it may be returned to the author for editing.
